Our case management services are designed to support adults struggling with mental health conditions by helping them navigate and access vital resources. We are dedicated to assisting individuals in achieving stability and fostering overall well-being through targeted interventions and continuous support.
Assistance in securing safe housing, accessing nutritious food, and obtaining clothing to meet basic needs.
Referral to healthcare professionals and mental health counselors for ongoing coordination of care.
Connection to educational programs, skills training, and employment assistance to promote lon term independence and self-sufficiency
Connecting individuals to local community programs and services that address various aspects of daily life, including social support and outreach.
Linking to peer support groups that offer emotional support, shared experiences, and valuable resources.
Referral to resources related to financial challenges, as well as legal aid for individuals in need of guidance on legal matters related to their mental health or living situation
Clients must be fully eligible for MaineCare.
Individuals must meet specific diagnostic criteria related to mental health challenges, or be at risk for homelessness, psychiatric hospitalization, residential treatment, or involvement with the criminal justice system. Alternatively, individuals must be members of a designated class or group that faces these risks.
